                        HEALTH CARE PARTNERSHIP PROPOSAL  

 

 

 

Dear Sir/Madam

 

I am writing to you to propose a health care partnership between myself, a practitioner of integrative medicine, and Healthcare Facilities. My specialty is providing healing touch services to persons suffering from cognitive/functional impairments, chronic pain, health care concerns and/or terminal illness.  My focus is to empower by encouraging patients to take a more active role in the healing process and thus reduce their dependency on the health care system.  I would like to work with your facility to improve their quality of life using integrative health care practices that are specific, measurable, achievable, realistic and timely.

 

Outputs

 

·        Direct results are a greater sense of well-being in the short term with actual measurable results emerging after a few sessions.

·        Long term results are monitored as we progress and can comprise measurable improvements in attitude, behaviours, skills and knowledge.

·        Patients suffering with any form of dementing illness such as Alzheimer’s may begin to demonstrate verifiable improvements in their cognitive capabilities – including memory retention.

 

Outcomes

 

·        Healing sessions and activities will result in specific and measurable results regarding client health with clearly defined and attainable targets. Targets will be different for each individual patient and remain within a realistic framework.

·        Individual patients’ improvements will be reflected within the care facility community as a whole, yielding system level changes that will result from program activities.

·        These will include group and individual healing sessions and workshop seminars in which clients will be taught how to empower themselves and follow up on input.

 

 

Impact

 

·        There will be an increase in overall patients’ health accompanying a parallel decrease in symptoms and suffering.

·        There will be a clear reduction in psychological stress relating factors as each patient becomes more aware of what is possible within the healing process. In some cases this will result in the elimination of illness.

·        Quality of life will be improved and measured not only through subjective patient testimonial but together with the patient’s treatment team.

·        The improved health status of the patients and resultant community will create a more self-reliant group thus culminating in a reduction of social inequalities. ·        The care facility will benefit through a direct reduction in cost per patient thus leading to a more harmonious environment overall.

 

Activites will begin with a seminar demonstration of how I work including a full explanation of methodologies comprising both theory and practice. I will then take a list of clients who wish to take part in individual sessions and those who might prefer a small-group option. A timetable will then be created and we will begin. The amount of time a patient spends with me will be individually assessed. Activities will include simple cognitive games such as memory work and music therapy practices if a piano is available.

 

My availability per site will be a minimum of two and a maximum of five hours per day. These hours will be inclusive of staff meetings and administrative duties.

 

Evaluation

 

·        Success will be evaluated at various stages against previously determined base line measurements.

·        Evaluation techniques can be modified to look at differentiated outcomes on a patient by patient basis.

·        Measurable indicators of success for outcomes will be both quantitative and qualitative. The former comprising measurements over sessions and the latter incorporating individual patient experiences together with staff appraisals.

·        Evaluations will use a simple ‘logic model’ based on Activities, Outcomes and Indicators of Success.

 

Budget

 

A realistic estimate of funding required must incorporate: salary requirements, program materials, administration costs, travel. I would prefer to discuss budgetary items in person but should you require a written proposal I would suggest a flat rate of $150 per hour or $50 per patient.

 

 

Sustainability

 

·        Plans to continue activities beyond any agreed upon time period will allow for follow-up work by Healthcare Facility staff and individual patients.

·        It will be incumbent upon myself to apply for grants for direct patient subsidization to obtain future funding that will sustain a synergistic partnership. Suggestions on the part of Healthcare Facilities would be welcome.

 

Conclusions

 

I am confident that Healthcare Facilities and myself can partner to provide healing services for patients that improve their quality of life and increase their tenure in a community like setting.  Throughout our collaboration I will seek strategic alliances with grant giving bodies in an effort to provide the best possible services to your organization and the people it serves.
